Are you taking medications including prescribed medicine, OTC medicine, vitamin or herbal medicine?　(薬[市販薬、ビタミン剤、漢方薬含む]は飲んでいますか？)

 

Do you have any allergy to medication or food?　(薬や食べ物へのアレルギーはありますか？) 

 

If yes, please list name of medicine/allergen and type of reaction.  (アレルギーの原因になった薬/食べ物と起こった症状を教えてください。)

Do you smoke tobacco? (タバコは吸いますか？)

 

How much do you smoke tobacco a day?  (一日にどのくらいタバコを吸いますか？)
